Dr. Suren Subramaniam consults in both Mandurah and Murdoch. He operates regularly at Peel Health Campus, St John of God Murdoch Hospital, Rockingham General Hospital, Waikiki Private Hospital, and St John of God Subiaco Hospital.

Please contact our friendly reception staff with your referral details and preferred location for a consultation. 

At the time of your first appointment

Please arrive 5- 10 minutes before your appointment for completing the new patient registration form.

If you prefer to, you could also complete and email the form before coming in for your appointment.

Please also bring the following for your consultation:

1. Referral Letter from your GP/Other specialists
2. Your Medicare card/ pension/ health care card details
3. Any Private health fund details
4. Any relevant x-rays / Ultrasound / CT scan /MRI results
5. Any reports of biopsies from GP or previous surgery results
6. A list of your current medications
7. If you have had surgery before, it is helpful to bring the contact details of your previous surgeon
8. If you have any significant medical conditions and see other specialists or doctors for these, bring as much information as you can as it may be relevant in planning for surgery
9. If you cannot speak English well and need help understanding or explaining yourself, bring along a relative or friend who can translate for you
